Student/Athlete Forms

All students participating in Co-Curricular Athletics at Ponaganset High School and Middle School must submit all forms to the Director of Athletics prior to competing in any practices or games.  

We are also excited to announce that the Ponaganset Athletic Department is utilizing Online Registration for all high school and middle school athletes. This process greatly improves the required Form process for Parents and Coaches alike.  ALL students interested in playing a sport must submit each form prior to participating.   In an effort to streamline the registration process we prefer that all parents utilize the Online Registration, but the paper form are still available.

In addition to the online registration, we must receive a current physical form from every athlete (within 1 year).  I've included our physical form with this email, but any doctor's form will suffice.  These forms can be turned into the Athletic Department mailbox in the office or Head Coach.

Parent Permission:  The Parent Permission form is an acknowledgement that parental consent to participate has been granted and agrees to the policies of the RIIL/RIPCOA, Ponaganset High School, and the Ponaganset Athletic Department.  This form is required for every season that an athlete competes. 

Physical Form: A current physical exam is required for all athletes.  Physicals are valid for one calendar year from the date of issue.  The Ponaganset form is preferred, but not mandatory.  Doctors may chose to issue their own Physical form, which must be submitted to the Director of Athletics.

Proof of Personal Insurance Coverage:  Each student must have medical insurance to compete in athletics.  The Proof of Insurance form annually certifies that the athlete has medical coverage through personal/family insurance.  The specific insurance company and policy number must be included on the form.  If a student is not covered by personal insurance through their parent/guardian, they must obtain the School Insurance to participate in athletics.

School Insurance:  School Insurance is offered to students and student-athletes as a primary insurance policy or to supplement their primary medical insurance.  Please return these forms directly to the Athletic Director.  If a student is not covered by insurance through their parent/guardian, they must obtain the School Insurance to participate in athletics.

Concussion Information Pamphlet:  This pamphlet must be signed by the parent and athlete annually to acknowledge they have received information regarding the effects of concussions.  

RIIL/RIPCOA Assumption of Risk:  The Rhode Island Interscholastic League (high school) and RIPCOA (middle schools) mandate that each athlete complete and submit this Assumption of Risk form, which certifies a student's home address and other information.  This form must be notarized and submitted once in the athlete's career.
